The insect is the Epeorus (E. Albertae). Here are three good imitations:

This bug is a clinger and emerges as an adult - SUBSURFACE. The Emerger and Spinner are the most important stages to imitate. I once sat on the bank, at 6:30PM, here:



Above $3.00 Bridge,

and was astounded by the number of very large fish swirling and slashing on rising emergers...
